FINANCIAL INFORMATION:

We strive to keep our fees reasonable; however, our fees due vary based upon the severity and complexity of your injury and illness as well as the time spent treating you.

Statements are sent to you on a monthly basis stating the unpaid fees.  Payment is due upon receipt of this statement, unless otherwise noted.  Payment is also expected even if a settlement is pending.

INSURANCE:

As a courtesy to our patients, we do provide the service of filing both primary and secondary health insurance claims for you as long as we have received current information.

Our office would advise you to check with your medical insurance company prior to your initial visit, if other than Medicare and Medicaid*, to ask the following questions about coverage on therapy services:

  1. Do I need a physician's referral?
  2. What coverage do I have?
  3. Is there a limit to the number of visits/treatments I can receive under my policy?
  4. Is there a co-payment amount?

*Medicare and Medicaid require a physician referral.

Remember: We are more than happy to file your insurance claims; however, we would like to remind you that regardless of your insurance or third-party liability coverage, you are ultimately responsible for all fees related to your care.
