Together, our staff strives to create a positive atmosphere for a caring, result-oriented and cost-effective environment.

You Have Chosen PTSR. Now What?

What to Bring:
When you arrive for your first visit, please check in at the front desk and provide us with the following:

  1. NEW PATIENT FORMS - completed
  2. VALID PHYSICIAN PRESCRIPTION - if you have one
  3. INSURANCE CARD(s)/PAYMENT INFORMATION
  4. CURRENT MEDICATIONS
  5. ANY OTHER SUPPORTING DOCUMENTATION - if applicable

Evaluation:
Now that we have your information, you will begin the initial evaluation process. This is a one-on-one session with your physical or occupational therapist to evaluate your condition and determine what treatment plan will be beneficial to you.

After the evaluation process, you will be asked to begin your treatment session. Expect this part of the evaluation and treatment to take between 1 and 2 hours. The length of all visits after this will be dependent upon your individual treatment plan and severity of your condition.

What to Wear
It is helpful to wear loose, comfortable closing and shoes. If you are being seen for a lower extremity injury/condition, please wear or bring shorts.
